Your First Visit
Your initial appointment will consist of a comprehensive examination. The exam is the critical information gathering that will enable us to develop the very best options for you to consider. Depending on the complexity of your case the exam may include;
- Photographs – Models of your teeth
- Medical Review – X-Rays
- Smile Analysis – Your Goals Desired
Dr. Nelson will explain your diagnosis and your treatment plan, which will have several options for your consideration.
Please provide the following information at your consultation;
- A complete list of over the counter and prescription medications you are taking.
- Your referral slip and any X-Rays
You may request that your previous dentist forward X-Rays to our office via mail. If the X-Rays are in a digital format they may be forwarded to [email protected]
- Insurance card
A parent of guardian must accompany all minor patients under the age of 18 at the consultation appointment.
